Minor Tattoo Policy

We tattoo minors aged 16-17 with a parent/ guardian present, a valid government-issued photo ID for both the parent/guardian and minor, and a completed, notarized FL Dept. of Health consent form. No facial, hand, or neck tattoos allowed for minors, regardless of consent.
